Tirzepatide: the 72-week trial and its body-composition substudy
SURMOUNT-1 randomised 2 539 adults without diabetes to three once-weekly dose levels or placebo for 72 weeks. Mean weight change ran from −15.0% in the lowest to −20.9% in the highest, against −3.1% on placebo.
N Engl J Med 2022;387:205–216 ↗A DXA substudy of 160 of those participants measured body composition directly: weight changed −21.3% on tirzepatide against −5.3% on placebo, and about 75% of the weight lost was fat mass and 25% lean mass — the same split as placebo.
Diabetes Obes Metab 2025;27:2720–2729 ↗The most common adverse events were gastrointestinal and concentrated in the escalation period; they caused discontinuation in 4.3%, 7.1% and 6.2% of the three dose groups.

Compared with Semaglutide 10 mg: Semaglutide is the nearest match by vial format and class. The documented difference is the measured endpoints: the tirzepatide substudy reported DXA body composition.
